DIVERT Scale

The DIVERT Scale is an output of the interRAI Home Care Assessment (RAI HC, interRAI HC) and interRAI Community Health Assessment (interRAI CHA with functional supplement).  The DIVERT Scale generates a score from 1 (lowest risk) to 6 (highest risk) indicating the client’s risk of a future ED visit.  The DIVERT Scale has 19 distinct outputs that fall under the 6 risk groups.  We targeted four subgroups with cardiorespiratory symptoms.


Who to Prioritize

 
  1. Start with the newest assessment - look at the DIVERT Scores within the last 90 days.

  2. Prioritize according to the highest DIVERT Score - 6 is the highest.

Alternative Method - All recent assessments with triggered Cardiorespiratory Cap then prioritize by DIVERT Score
