Cardio-Respiratory Model

The Cardiorespiratory Model is comprised of 8 components.  Built on the principles of transitional care and integration, the components work to improve access to health promotion and disease prevention community resources, improve communication and collaboration, and equip the person with chronic disease management skills. 

The intervention can be offered to those clients who present with a DIVERT Scale Score from 3-6 with accompanying cardiorespiratory symptoms. Each component works together to provide a comprehensive person centered approach to care.

 At the center of the intervention is targeting those who would benefit most, making this intervention an efficient use of health care resources.
