SBAR Communication Tool

SBAR is a standardized communication technique using the Situation, Background, Assessment, and Recommendation for framing a focused concise communication of the problem or area of concern.

The SBAR format improves information transfer.  SBAR communication was used to communicate with primary care providers, specialists, and the health care team.
